- to participate in KetoBasic and KetoPremium, the intake form and an informed consent must be submitted no later than 48 hours before your scheduled intake appointment. If this condition is not met, the appointment will be cancelled and a cancellation fee of €25 will be charged
- consultations within a programme can be rescheduled a maximum of 1 time, after which the consultation will be cancelled
